Appointment Forms
Once you have made an appointment at Counseling for Faculty and Staff by contacting us at 828-262-4951, we will ask you to fill out two simple forms prior to your first visit. You may come in 10 minutes before your appointment is scheduled to fill out these forms in our office or, if you prefer, you can download them below, fill them out, and bring them with you.
An Informed Consent form simply advises you of our policies and procedures. In particular, it describes federal and state exceptions to complete confidentiality.
We also ask you to give us some basic information about yourself in the Client Information form.
